‘I Wasn’t Able to Even Open My Eyes. That’s How Swollen I Was’

Onemanda Preisinger is nervous about her daughter’s upcoming 13th birthday party. Not for the typical causes associated with a home packed of loud adolescent kids, but because it’s the first time she will showcase her recently altered face to acquaintances and extended family. “Obviously I’m going to inform all guests as they come in, ‘Just so you know, this is not how I appear,’” says the 30-year-old real estate agent from south Florida.

How she looks is, well, a little startling – her face swollen and preternaturally lifted, as though held together by heavy-duty tape. Her altered – and she’s keen to stress, short-term – appearance is the outcome of six aesthetic surgeries, such as an minimally invasive facelift, conducted by a surgeon in Istanbul, Turkey, last month. “My spouse became emotional when he saw me for the first time because I couldn’t even open my eyes. That’s how puffy I was,” she tells me via online call from her home. “I needed to inform him: ‘Honey, I’m fine, I’m not hurting. I just look like someone jumped me.’”

Increasing Popularity of Early Facelifts

According to cosmetic specialists, Preisinger is among a rising count of individuals choosing to undergo a rhytidectomy in their 20s and 30s – significantly before a ten years before most doctors’ typical patient age range of 40 to 60. (Although most surgeons are eager to highlight the industry edict of: “We treat genetics, not years.”) “I have 28-year-olds requesting facial lifts,” says London-based aesthetic plastic surgeon Georgios Orfaniotis, a ex-NHS doctor who focuses on the head and neck. “It’s a major surgery, and I’m a bit concerned when I observe individuals opting for it as a lifestyle choice.”

Explaining the Facelift Procedure

A facelift, also known as a rhytidectomy, elevates the tissues in the face that begin to droop as we age. “Our faces are structured a bit like layers of an onion,” says Kent-based facial plastic surgeon Marc Pacifico. “Dermis and adipose on the top, then a layer of connective tissue known as the SMAS. Think of the facial framework as the soft tissue skeleton of the face. And that’s what causes aging; that is what loosens and sags and pulls the dermis down with it.”

Previously reserved of moneyed older people, “filler fatigue” – when excessive use of dermal fillers expands the face and leads to laxity in the skin – combined with the common adoption of slimming medications, cut-price surgical travel, and the development of novel, famous-figure-promoted operative methods are driving a different type of patient towards this major operation. Reports show an eight percent rise in facial lifts over the past 12 months in the United Kingdom; while a study found that the percentage of youthful candidates is increasing, with one-third of procedures now conducted for patients aged 35-55.

“Patients are now requesting to appear as the optimal form of themselves and naturally the techniques are evolving,” comments Orfaniotis. At present, the operation du jour is the deep plane facelift, a technique promoted by figures including reality TV star Kris Jenner, to fashion designer Marc Jacobs.

Advanced Lifting Method

While a more traditional facelift entails lifting the superficial layer, the deep plane facelift releases the facial ligaments underneath, allowing doctors to reshape the face by moving the deeper tissue, and preventing the taut, pulled appearance sometimes wrought by older methods. “We’re not putting tension on the dermis because we’re targeting deeper, to the deeper tissue,” explains prestigious London plastic surgeon Rajiv Grover, who specialises in the advanced method. “It is the deeper tissue that is repositioned. The dermis comes with it as a passenger.” Elevating the whole facial structure as a unified block results in a authentic-appearing and longer-lasting result. It additionally implies that the procedure is not just being used for the express purpose of youth preservation – or revitalization, as the cosmetic field terms it – but for “beautification” and contouring, too. “We’re getting numerous requests from clients aged late twenties to mid-thirties for this reason,” notes Grover.

Individual Experience

Preisinger had never planned on getting a facelift – at least not at the age of 30. But years of using hyaluronic acid dermal fillers in an effort at “facial balancing” implied that by the time she reached her late twenties, she was “damaged”. “I didn’t do this because I desired to appear youthful,” she clarifies. “I underwent it because the injectable ruined my face. I regret ever using it. If I had avoided the injectable, I don’t believe I would be where I’m at currently.”

If injectable substances ever fully dissolve has historically been a debated topic in the cosmetic field. “Studies have shown that not only do they rarely completely dissolve,” says Grover, “but they migrate and can obstruct lymphatic channels. A contributing factor to what we call ‘puffy appearance’ is also the fact that in a sense, the face gets slightly waterlogged because its ability to remove bodily fluid has been reduced.” Though studies into the area is early-stage, cautionary statements on dermal fillers’ possible effects on the body’s drainage have been issued, and further research initiated. An esteemed face specialist, commenting on the condition of anonymity, mentioned he would never use fillers in a client because of the dangers they present. “A lot of doctors avoid talk about this, because the companies who make injectables can be pretty aggressive.” He’s been told accounts, he adds, of cosmetic doctors facing court orders after raising their concerns.

Choice and Surgery

For Preisinger, once she started using injectables, she felt as if she needed to continue adding more – particularly when it started to migrate to other parts of her face. She was just 28 when a nearby cosmetic doctor in the state suggested that a facial and neck surgery could be what she needed to exit the injectable hamster wheel. Two years later, she ended up selecting from a list of suggested accommodations in the city, texted to her by the clinic assistant at her doctor’s clinic.

Initially, Preisinger had planned for an eyelid surgery – better known as eyelid surgery – but her doctor recommended that a facial lift was the correct option for the looseness she was experiencing in her face. She arranged a forehead lift, a cheek lift and a neck lift. The day before her operation, one day after she’d touched down solo in Turkey, she decided to add a mouth enhancement. “Then he stated, ‘We will lift the lower eyelid puffiness.’ He added, ‘I think if we remove some buccal fat, it will sculpt your face.’ So I ended up adding two more surgeries,” she recalls. She spent $22,000 (sixteen thousand five hundred pounds) for the half-day surgery, plus a three-day|
